How to Apply

The Application Form, Spouse's Supporting Application and Reference Form are linked to this page. Download the form(s) to your computer, fill out (please note that some boxes are set up to expand as needed for multiple lines), print on your printer, sign, and mail to the address below. Four (4) references are required and should also be received in our office by the deadline date.

Due to the nature of the Oklahoma Agricultural Leadership Program, targeted applicants are those between 25 and 45 years of age. This does not preclude consideration of other applicants. The participation fee is $1,500. In some cases, full-time producers may qualify for a waiver of a portion of this fee.

There are three (3) possible steps to the application process:

1. Application Form to be completed by program applicant.
   
   (Due to the length of the document, we suggest you download/save to your computer and complete at your convenience.)
   (Both applicant and spouse, if applicable, must sign form.)



2. Supporting Application Form to be completed by spouse (if applicable).



3. Four (4) References are required. Reference Forms to be submitted by persons of your choice, one of which should be from a banker/lender or someone who is familiar with the financial aspects of your farming or business practice. Please note that this form can be filled out 1) onscreen , 2) downloaded/saved to your computer to complete, or 3) printed and completed manually. Reference Forms are to be mailed to our office directly by the reference person (do not include with your application). We suggest that you include a stamped envelope addressed to our office when you ask someone to complete the Reference Form.
